Moraine was told that Lanfear was killed after draining her of the One Power.
Since we see that Moiraine was nearly drained, it seems more likely that Lanfear escaped after only being slightly drained, hence her reduced potential as Cyndane.
Since we now know conclusively that Lanfear did not kill Asmodean, and that Moiraine was granted 3 wishes:
1) What do you think Moiraine's other 2 wishes were?
2) What were Lanfear's 3 wishes?

Her soul was transmigrated into a new body after she died.
Most likely, the Eelfinn killed her by draining her OP capacity too quickly, but they weren't able to drain everything before her death. So she had a reduced potential when the DO took her soul and put it in a new body.
