And, most of all, she has failed to learn humility. That is the mistake of her predecessor in the AoL, and without that she will fail to stop a second Breaking (through tainting of the source).

I also found her answers about Rand to be (while better than the average AS answer) rather lacking, if not completely off the mark.

Semirhage's pathetic nature is explicable in light of Verin's nature. The DO doesn't want his tools to be too clever. Graendal tried that and was used as a pawn.

In fact, I think Semirhage's demise had more to do with the DO or Moridin wanting to be rid of her (even before her capture) than any action be the Lightsiders.
