This being a DARK prophesy, this is from the POV of the DO, the original ancient wrong could be the creator sealing away the DO at the moment of creation. the original slaying of the ancient wrong is the boring in to the prison of the DO, releasing the DO.

conclusion... "Again" slaying of the ancient wrong means the RE-release of the DO via breaking all the seals.

i think this fits the best, the seal has to break before the Dark Lord comes.
