Given the overdraft fees he incurred, it could be that there were other prior purchases that weren't accounted for/deducted yet. Or it could be that the amount the bank wanted to "hold" for the purchases (which typically exceeds the amount necessary for the actual purchase) was more than the $19 he thinks he had in his account at the time.
Again, though, not disagreeing that tracking is essential, personal responsibility is good, and coming down hard is warranted
