Most of the ending (whether you get green or not, if people step out of Normandy, etc) depends on your War assets and readiness rating. Also if you have high paragon you can save both Quarians and the Geth, but you also need certain amount (don't remember how many) of points from quests involved with Rannoch and the Quarian-Geth conflict.
By the end of the game my paragon was pretty much as high as it can be. Maybe it wasn't enough yet during said quest. I noticed I didn't get to safe that general anymore because I chose the other quest on Rannoch first. Maybe a replay is in order.
Yeah, I think the General has to be saved. Also, rewriting/destroying the Heretics from ME2 does have impact. Destroying grants you points toward the co-operation goal.
