Of my squad: none, and Shepherd survived. Kelly got goo'd and I lost half the ship crew though, according to Dr. Chawkwa. It might be worth noting that I had all ship upgrades and a completely loyal team.

Also - I didn't tell anyone to hold the door before the final approach. I chose the option to go all-in for the that scene, and had two of the squishiest characters with me at the time. (playing Vanguard, Veteran difficulty) I also blew the final boss's canisters two at a time, so it was pretty quick.
The sword might be grateful to the forge fire, but never fond of it.
