There are two things that could have caused it, as far as I know:

1. Mordin wasn't loyal.
2. You made the "wrong" choice of person to lead the other team during that scene. Jacob, miranda, and Garrus I know are good choices (and there might be more). But some characters, when chosen as leader of the fire team, cause people to die. And it's not always obvious that that's why, either.


He was loyal and I had Garrus as team leader 2. Hmm.
