Thom and Moiraine getting together is something that is alluded to and hinted at, but never spoken plainly. So, no, you wouldn't recall Min outright saying that the vision that she got wrong was the Thom and Moiraine one. It's one of those things you have to figure out yourself from the clues provided. The Wheel of Time series has a lot of prophecies and visions like that.

When Min is thinking about how Rand needs "a woman dead and gone" and that Min got a viewing wrong, in the latter statement she is actually referring to the fact that Moiraine "died" before her and Thom hooked up. Again, you're just going to have to trust me (and Cannoli) on this one. All the pieces of the puzzle are there, but the reader isn't being slapped in the face with complete disclosure.
