I forget the exact passage, but I feel like Siuan implied that she somehow ordered it.

She felt responsibility for it, since after they came back from Falme, she sent them out again instead of keeping them in to gradually train them. That's the time period where Egwene went from being around as strong as Siuan to reaching her full potential.
