Just because you received a refund does not mean you have a no income tax or negative income tax. To be counted as the 47% you must receive some sort of direct financial credits or wealth transfers that were greater than your total income taxes paid. Thus things like earned income tax credit, children tax credits, food stamps count but things like medicare spending do not.
