That monarchy is inherently unstable. After all in real life history in the middle ages, 50% of people who were born did not make it to the age of 15. The "certainty" of a heir, and the right age to inherit and build social capital with the next generation of lords and retainers is not guaranteed.
Add into the fact the 4 different ways to do inheritance via progeny (various forms of primogeniture) and you make an uncertain system even more complicated.
But most importantly layering onto this unstable system that there is no way to know if the next generation of leaders will be "virtuous", they can be vain, they can literally be insane, they can be spiteful little shits, dramatic and erratic, etc, etc.

In Monarchy you may not be "blessed" to have anyone sympathetic to root for, it may be the Dune series all the way down. And this is why Martin tricks you with the first book, by creating Ned Stark as
Duke Leto Atreides point of view surrogate one has the desire to root for the good guys, only to subvert it through the journey and let "inertia" carry you forward, for now you are hooked, you are addicted and invested even if one can also HATE the series now.
