I think the ter'angreal melted because two people went through at exactly the same time, which is not the way the ter'angreal was supposed to be used. Furthermore, since the ter'angreal had MELTED, there was no way to return and so the connection with the Randland world was severed (which explains Lan's bond disappearing).
