The implication of Lan's comment to Nynaeve that they are not southlanders and mention of her coronation, is that in Malkier the wife of the King is Queen in truth and not merely queen-consort. The suggestion is that they reign as equals.

As far the reliability of the test, those have been shown to be fairly accurate on issues of the setting, if not people. Egwene learned the truth about converting channelers to the Shadow and Nynaeve seems to have picked up some accurate details about Malkieri fashion, so maybe Lan had the royal customs right. The fact that the queen get's an honorific "el" supports this, IMO.
