Why is Umbridge smiling as Harry exits the examination room after having produced a Patronus?
Most likely to tell Harry that he can be as awesome as he likes in DADA, he'll never be an Auror or any other prestigious profession if she can help it. Which as it turns out she can't, but she doesn't know that.

In the dream/vision Harry has during the exam, he is described as "jogging". I cannot for the life of me imagine Voldemort jogging. He should have taken that as a sign the vision was false. Jogging, I ask you.
Good point. The whole vision is kind of absurd, really, for the reasons Hermione mentions and others.
Yes. I suppose that is a good thing. A reader cannot really complain about having been tricked unfairly.
Bless Hermione for asking the sensible question of how Voldemort could possibly be in the Department of Mysteries. It really isn't the most sensible place to be torturing someone.
Not to mention that Sirius is apparently there unguarded until Voldemort "jogs" in to torture him.

But. The fact that all these sensible objections are aired, and then spun around with the plausible (more or less) explanations of Ron and Harry (some of which have been building since day one -- Sirius' desire to get out of the house, for example). It is very convincingly done.
Plus the simple expectation of the reader that a big showdown in the DoM is about to happen, and it makes sense for Sirius to be involved in that.

And her desire to verify is what makes it all so much more convincing. Her doubts mean that when she finally accepts it, we have to as well. Despite the loophole that Kreecher does not have to tell the truth to Harry. That is too easy to forget.
Do we find out later why Sirius doesn't show up himself and Kreacher can feel so secure in lying without the risk of Sirius walking in to prove him wrong? We probably do.
Isn't Sirius up feeding Buckbeak or something?
I suppose this, in a way, justifies the need for the earlier use of Umbridge's fire as well. It needs to have happened before for it to make sense for Hermione to jump to it as a plausible way of verifying the dream in such a pressed situation.
Yeah. For her having guarded it so closely, though, apparently the attacks by those digging animals are the cause - not Harry's earlier attempt after all.

I like that we get the explanation for the Dementor attack, and that it is not what we would have expected. It is not, in fact, proof that the Ministry is losing control.
It is losing control by the time we find out about it, though. Just a delay of the inevitable.
