It makes sense for when there are multiple sith at a time (note the rule of two didn't occur until after the Old Republic games, and in the last 100 years after the two trilogies it has been broken completely), two things happened

1) The sith killed each other due to infighting
2) When there was multiple sith the jedi did their best to hunt down the sith.

Now since the rule of two, the jedi believed the sith to be extinct. Furthermore the apprentice never challenges the master till he is confident he will win. If he fails the master kills him for he is too weak and thus not a worthy successor and thus tries to find another apprentice.
