i always figured it was pretty much your soul that you dreamt into T'A'R, hence the reflecting injuries when you woke...
You could certainly die of shock. TAR is metaphysically different from normal dreaming as the dreamer is more solidly there.
I've also heard of experiments done that involve blindfolding a patient, pouring warm water down their arm and telling them that they're bleeding to death. End result, the patient died.
Another involved putting a patient into a clean, new hospital bed, and then lying and telling them that the former inhabitant had died of cholera. The patient then exhibited the symptoms of cholera and died themselves.
these results indicate a sort of psychological death, tricking the mind into thinking it's dying. the patient who was told he was bleeding to death didn't actually manifest a gash in their arm, their brain told itself it was dying and so did (btw i'd definitely need more solid evidence than "i've also heard of experiments..." to make a proper response)
whereas if you injure yourself in T'A'R the injury appears in the flesh too. Their minds couldn't physically damage their bodies like that. Therefore it seems most likely that it's more substantial than just a creation of the mind
nor could other particularly nasty things happen to you
