I think that this prophecy refers to Justice, Egeanin, and Tuon:

TITLE - The Shadow Rising
CHAPTER: 21 - Into the Heart

Into the heart he thrusts his sword,
into the heart, to hold their hearts.
Who draws it out shall follow after,
What hand can grasp that fearful blade?


I believe that Egeanin/Leilwin will draw Justice out of Gawyn's body and redeem herself by presenting the sword to the Empress (not because she wants to redeem herself, of course, but because she honestly believes that it is the rightful property of the Empress). The sword will be a symbol of unity that Tuon will use to lead the forces of Light against the Shadow, and Egeanin will follow after her. The prophecy avoids being gender-specific.

This leads to Tuon being Egwene's woman with the sword, and also probably the Seanchan woman that is with Egwene when Rand confronts her, which I assume will be after he is ripped out of Tel'aran'rhiod. I assume that because according to Nicola's Foretelling, the truce won't happen until after Rand dies.
