Are you referring to Shien Chunla? The Hall was unable to prevent wars. They didn't start them. Might as well call it an atrocity of the UN that it cannot prevent all wars.

Wars were all over the map during her reign, because of the mess the Hall made of the responsibilities the Tower has. And even as those wars raged across the nations, the whole Tower didn't do a thing about it, untill they became fed up with the internal mess and bickering.
In the end Chunla was murdered by her former colleagues.
