Remember Egwene? She was held prisoner in the Stone, entered T'A'R, accomplished a Shield on a Black Sister and returned to her body only to find that the physical door was still locked. Moghedien could have done the same.
Especially since the most important information is already revealed to her: Nynaeve had forced Moghedien to enter T'A'R to go to Caemlyn to fight Rahvin, thus Moghedien could have been busy in T'A'R while sleeping, trying to find a way out of her predicament.

But Asmodean wasn't killed in the Dream World. He was killed in the real one. I know that the adam doesn't limit one from entering the real world, but ultimately what does it matter. You cannot effect the real world from the dream world. If it was at all possible, Moghedian would have known about it and done it to get herself out. Instead, she took her punishment and ultimately dealt with the consequences.
Also remember that Nynaeve didn't force Moghedien into the Dreamworld....she caught her there. It wasn't until Moghedian was looking for a way to win her freedom with info that Nynaeve even knew that Rand was battling Rhavin. It was then just blind luck/ tavaren that the end of the battle happened to go down in the Dreamworld & not the real one. Otherwise, Nynaeve linked to Moghedian would have been completely useless.

The lead up was clumsy because her death scene was rather forced.
