IMO she actually made very few real mistakes, but she was walking a very thin line and it didn't take much to topple her, and Leanne got sucked in for the ride.

Siuan's main errors (IMO):

1. Secrecy, despite her fears of the BA she needed to tell the Hall something about the DR.

2. Arrogance, even knowing how tenuous her situation was she still ignored Min's viewing and changed nothing of her behavior and took no precautions.

3. Web of AS lies, like Moghedien Siuan got caught in her own web of deviousness ... had she not forced BA Hunting on the girls Elaida wouldn't have been sent packing from Caemlyn for reasons she could lay on Siuan. Couple that with her keeping Min around to "read" people and openly manipulating Gawyn and Galad in regards to Elayne's vanishing and you have a recipe for disaster.
