It simply makes no sense that - of all people - Graendal (who was even easily outsmarted by Sammael) achieved that this late in the series, so that Rand still has to deal with her in an ever smarter way in the next books.
So how did Sammael outsmart her? She did outlive him for one, and she knew what he was up to for two.
Lanfear and Asmodean did outsmart Rand. Rand was completely caught off guard by the Dragons on Couldin's arms. But beyond that, he was completely wrong on who Lanfear was posing as.
But here's something to ponder...how much of Rand's success is based upon his awsomeness, and not on ta'varen twisting of fate? Rhavin would have killed Rand, if Nynaeve didn't act. An action she could only have done if they went to the Dreamworld in the flesh...something that Rhavin just happened to do.
Lanfear would have killed Rand in her own suprise attack if not for Moiraine's actions.
Rand would have been killed by Mashadar in Shadar Logoth if not for Moridin's intervention. Sammael's gamble to take the fight there would have worked.
Contrary to popular belief, I don't consider the Forsaken bumbling fools. Instead they are a house divided against itself. If they worked as a team...for the good of all and the glory of no individual, they would have rolled over the forces of the light. Let us remember that back in the War of Power, the Light was loosing. Hence the despiration plan of LTT.
